Care delivered in your home, a skilled nursing facility, or assisted living — wherever you’re most comfortable.
Palmetto Hospice & Palliative Care is a locally owned and operated practice based in Tifton, Georgia. We believe every patient deserves individualized attention from a team that truly cares.
Our interdisciplinary team of nurses, physicians, social workers, chaplains, and trained volunteers works together to address the physical, emotional, and spiritual needs of each patient and their family.
We accept Medicare, Medicaid, and most private insurance plans. Most hospice services are covered at little or no out-of-pocket cost to patients.
We provide a full spectrum of hospice and palliative services, tailored to each patient’s unique needs and wishes.
Regularly scheduled visits from specially trained registered nurses to monitor symptoms, manage medications, and coordinate care plans.
Expert management of pain, shortness of breath, and other discomforting symptoms to maximize quality of life and patient comfort.
Compassionate social workers assist with financial guidance, insurance coordination, advance care planning, and emotional counseling.
Spiritual support tailored to the patient’s beliefs, coordinated with their own clergy or provided by our trained pastoral counselors.
Home health aide assistance with bathing, dressing, grooming to preserve comfort and dignity at home.
Ongoing grief counseling and bereavement resources for families before and after the loss of their loved one, for up to 13 months.
Our practice is led by experienced healthcare professionals who bring clinical expertise and heartfelt compassion to every patient relationship.

We follow a holistic, team-based model that centers the patient and family in every decision — from the first conversation to the final goodbye.
We begin with a thorough assessment of medical, emotional, spiritual, and social needs — then build a care plan that reflects each patient’s wishes and values.
Physicians, nurses, social workers, chaplains, and volunteers collaborate closely to address every dimension of the patient’s and family’s experience.
We treat the family as part of the care team, providing education, training, respite care, and emotional support throughout the illness and beyond.
Our care team is available 24 hours a day, 7 days a week — because serious illness doesn’t follow a schedule, and neither do we.
